The Aeronautical Accidents Investigation and Prevention Center (Portuguese: Centro de Investigação e Prevenção de Acidentes Aeronáuticos, CENIPA) is a unit of the Brazilian Air Force that investigates aviation accidents and incidents in Brazil. It is headquartered in Brasília.

Space accidents are investigated by the Space Activities Accident Investigation and Prevention System (Portuguese: Sistema de Investigação e Prevenção de Acidentes em Atividades Espaciais, SIPAE).

On September 29, 2006, Gol Transportes Aéreos Flight 1907, a Boeing 737-800 on a scheduled domestic passenger flight from Manaus, Brazil, to Brasília and Rio de Janeiro, collided mid-air with an Embraer Legacy 600 business jet flying on an opposite heading over the Brazilian state of Mato Grosso. The winglet of the Legacy sliced off about half of the 737's left wing, causing the 737 to break up and crash into an area of dense jungle, killing all 154 passengers and crew on board.

TAM Airlines Flight 3054

TAM Airlines Flight 3054 was a regularly scheduled domestic passenger flight operated by TAM Airlines from Porto Alegre to São Paulo, Brazil. On the evening of 17 July 2007, the Airbus A320 serving the flight from Porto Alegre overran runway 35L at São Paulo's Congonhas Airport after touching down during moderate rain and crashed into a nearby TAM Express warehouse adjacent to a gas station.

2009 Manaus Aerotáxi Embraer Bandeirante crash

The crash of an Embraer EMB-110P1 Bandeirante owned and operated by Manaus Aerotáxi occurred in the state of Amazonas in Brazil on February 7, 2009. At 13:50 local time, a Manaus Aerotáxi Embraer EMB-110P1 Bandeirante twin turboprop, registration PT-SEA, operating as an air taxi flight from Coari to Manaus (SBEG), crashed into the Manacapuru River about 80 kilometers (50 mi) southwest of its destination, killing the two crew and 22 of the 26 passengers on board.
